Job Summary:
The Associate Medical Director will work closely with primary care providers, care management teams, and executive leadership to build and scale a comprehensive behavioral health program serving:
Homebound patients- Assisted living residents
- Patients with dementia and cognitive impairment
- Patients with complex psychiatric and medical comorbidities
This role is ideal for a physician who enjoys building programs, improving systems of care, and mentoring clinicians, while maintaining meaningful clinical engagement.
Responsibilities:
Clinical Leadership
- Provide clinical leadership for behavioral health services across Intention Healthcare’s home-based and facility-based programs
- Develop and implement evidence-based behavioral health care models integrated with primary care and long-term care
- Establish clinical protocols and treatment pathways for psychiatric and behavioral conditions common in medically complex populations
- Collaborate with medical leadership and facility partners to improve behavioral health outcomes
- Ensure quality, safety, and regulatory compliance across behavioral health services
Clinical Supervision
- Provide clinical supervision and mentorship for Behavioral Health Nurse Practitioners and other behavioral health providers
- Review complex cases and support treatment planning
- Lead regular case review meetings and interdisciplinary discussions
- Ensure providers are practicing according to evidence-based behavioral health standards
Program Development
Design and scale a comprehensive behavioral health program supporting both home-based and facility-based patients
Develop care models addressing:
Depression and anxiety
Behavioral and psychological symptoms of dementia
Serious mental illness
Substance use disorders
Caregiver stress and behavioral health needs
Support implementation of programs such as:
Collaborative Care Model (CoCM)
Behavioral health integration
Tele-behavioral health services
Work with operations and technology teams to develop scalable workflows and documentation processes
Direct Patient Care
- Provide psychiatric evaluation and treatment for patients through telehealth, in-home visits, and facility-based care
- Manage complex psychiatric cases in collaboration with primary care providers and facility staff
- Prescribe and manage psychiatric medications as appropriate
Collaboration
Work closely with:
Primary care providers
Behavioral health clinicians
Care management teams
Social workers
Family caregivers
- Provide education and guidance to interdisciplinary teams on behavioral health topics
Qualifications:
Required
- MD or DO with Board Certification in Psychiatry
- Active medical license (or ability to obtain licensure in relevant states)
- Experience treating adults with complex psychiatric and medical conditions
- Experience supervising behavioral health clinicians (e.g., NPs, PAs, etc.)
- Familiarity with documentation and billing requirements for behavioral health services, including psychiatric evaluation and management services, psychotherapy codes, and behavioral health integration models
- Understanding of regulatory and compliance considerations for behavioral health services in outpatient, home-based, and long-term care settings
- Strong interest in integrated care models that combine behavioral health with primary care
Preferred
- Experience in geriatric psychiatry or long-term care psychiatry
- Experience with Collaborative Care Model (CoCM) or Behavioral Health Integration
- Experience with home-based care or telehealth
- Leadership or program development experience
- Familiarity with value-based care models
