JOB OVERVIEW
This position offers short-term support to families experiencing trauma, abuse, instability, or other challenges. The role involves assessing individuals' specific needs, developing personalized service plans, and connecting clients with community resources for ongoing support to ensure their safety and promote family preservation. Key responsibilities include advocating for clients, monitoring their progress, and maintaining accurate records.
Key Responsibilities
- Assessment & Care Planning: Conduct intake assessments to identify client needs, barriers, and goals, subsequently creating individualized service plans
- Resource Connection: Connect families with essential community resources, including housing, mental health, legal aid, and public benefits.
- Crisis Intervention: Provide immediate support and intervention during emergencies to ensure the safety of the child and family.
- Case Monitoring: Regularly check in with families to evaluate their progress, adjust care plans as needed, and ensure they remain engaged in required services.
- Documentation: Maintain accurate, timely case notes, track outcomes, and fulfill reporting requirements using client management software.
- Advocacy & Collaboration: Act as a liaison between families and external systems (schools, courts, medical providers, and law enforcement).
- Education & Group Facilitation: Lead parenting skills class.
Qualifications and Skills
- Education: Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
- Experience: 1–2 years of experience in social services or case management.
- Skills: Excellent interpersonal communication, strong organizational abilities, cultural competency, effective teamwork, proficiency in Microsoft Suite, exceptional documentation skills, and experience working with diverse and high-risk populations.
- Requirements: A valid driver's license, reliable transportation, and successful completion of background checks.
Pay: From $54,361.00 per year
