Job Summary
Shift: Friday, Saturday, Sunday, Monday 8:00am to 6:00pm
License Required: PA RN License
Mileage Reimbursment: 0.62 cents/mile
Counties Covered: York and Lancaster counties
Required experience: Hospice and HCHB experience required. Candidate will be required to complete admission, routine, PRN, and death visits.
Productivity benchmark 6 to 8 visits per day.
RN License, Valid Driver's License, Valid Car Insurance, Valid BLS CPR Required
Duties:
- Conduct comprehensive nursing assessments, develop and implement individualized care plans, monitor patient progress, and promote health maintenance, disease prevention, and case management.
- Collaborate with physicians and interdisciplinary healthcare teams to coordinate care, manage medications, communicate changes in patient condition, and ensure timely follow-up on health-related appointments and hospital discharges.
- Maintain accurate clinical documentation, including nursing notes, physician orders, progress reports, and required daily, monthly, quarterly, and annual records.
- Ensure compliance with medication management policies, infection prevention standards, and healthcare regulations while maintaining a safe environment for individuals served.
- Supervise and train licensed and unlicensed staff, provide health and safety education, oversee nursing support activities, and ensure competency in person-centered care practices.
