Position Overview
We are seeking a Transition Care Coordinator to support members transitioning
between levels of care, including hospital discharge, community placement, and long-
term services.
This role focuses on care coordination, service linkage, and ensuring members receive
appropriate medical, behavioral, and social support services.
Key Responsibilities
Coordinate transition care services for Medicaid members
Conduct assessments and identify member needs
Develop and implement transition care plans including risk mitigation plan
Connect members to medical, behavioral health, and community resources
Communicate with providers, case managers, and family members
Monitor member progress and update care plans as needed
Maintain accurate and timely documentation in compliance systems
Support safe discharge planning and continuity of care
Submit financial and community service program applications on behalf of
members.
Qualifications
Bachelor’s degree in Social Work, Nursing, Public Health, Human Services, or a
related field OR 5+ years of experience working with Medicaid populations, care
coordination, case management, or social services
Experience in care coordination, case management, or social services preferred.
Knowledge of Medicaid and/or transition care services preferred
Strong communication and organizational skills
Ability to manage multiple cases independently
Basic computer and documentation skills
Valid driver’s license
Ability to travel to member locations as needed
Preferred:
Experience with elderly, disabled, or high-risk populations
Familiarity with Colorado Medicaid or transition care programs
Work Schedule
Monday to Friday
Up to 40 hours per week (flexible scheduling)

$25/hour
