Bayhealth, Inc. is seeking a per diem Home Health Physical Therapist Assistant to serve our patients. As a Physical Therapist Assistant, you will provide essential home health services under the guidance of a Physical Therapist. Your role is key in helping patients relieve pain, restore function, and achieve optimal health outcomes within the home setting.
Carry out physical therapy treatments as directed by the supervising Physical Therapist, aligned with patient goals and physician orders.
Provide hands-on care, including therapeutic exercises, gait and functional training, and muscle reeducation.
Utilize physical agents (heat, cold, ultrasound, etc.) to support treatment goals.
Observe, report, and document the patient's response to treatment, coordinating closely with the supervising Physical Therapist, Clinical Team Manager, and other team members.
Instruct patients and their families in home exercise programs and the use of assistive devices.
Maintain timely and accurate clinical records, contributing to patient progress notes, care documentation, and interdisciplinary conferences.
Adhere to agency standards for using telecommunication and point-of-care technology.
Participate in interdisciplinary team meetings and in-service education programs to continually enhance home health care standards.
Engage in quality improvement initiatives and support agency protocol development as needed.
