Overview
We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for our patients. This position requires strong clinical skills, a commitment to patient care, and the ability to work collaboratively within a multidisciplinary team. The Pharmacist will be responsible for providing pharmaceutical care in various settings, including hospital medicine, managed care, and critical care environments.
Responsibilities
- Review and interpret physician orders to ensure accuracy and appropriateness of medication therapy.
- Conduct patient assessments to evaluate medication needs and monitor therapeutic outcomes.
- Provide drug information and education to healthcare professionals and patients regarding medication therapy.
- Collaborate with healthcare teams in managing complex medication regimens, particularly in ICU settings and Level I Trauma Centers.
- Prepare and dispense medications, including IV infusions, ensuring compliance with safety standards.
- Participate in pharmacy residency programs to mentor students and new pharmacists.
- Maintain accurate records of medication dispensing and patient interactions in accordance with regulatory requirements.
- Stay updated on advancements in pharmacotherapy, hospital medicine, physiology, anatomy, and managed care practices.
Qualifications
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Active pharmacist license in the state of practice.
- Completion of a pharmacy residency is preferred but not required.
- Knowledge of hospital medicine practices, patient assessment techniques, and managed care principles.
- Strong understanding of physiology and anatomy relevant to medication management.
- Experience working in an ICU or Level I Trauma Center is advantageous.
- Excellent communication skills with a focus on patient service and collaboration within healthcare teams.
- Ability to work effectively under pressure while maintaining attention to detail.
