A Nurse Practitioner who works under the direction of the Associate Medical Director (in matters of clinical practice) to provide patient care services to patients within the Outreach and Priority Care Clinics.
Professional Duties
- Utilize proper channels to effectively communicate verbally and in writing.
- Utilize electronic medical/health record system for documentation and coding.
- Understands the importance of HCC Coding standards and documents accordingly.
- Collaborate with the department director and/or VP, Clinical Services Integration to address urgent issues.
- Understand seniors’ needs and endeavor to accommodate as need and when possible.
- Actively promote End-of-Life decision making.
- Understand productivity standard expectations.
- Attend and participate in meetings as requested.
- Perform other duties as assigned.
- Serve as an ambassador for Desert Oasis Healthcare and Family Hospice Care at all times and positively shape the customer experience
- Let your patients, customers and co-workers know what they can expect from you and follow through
- Summarize key information and provide written materials whenever possible
- Speak positively about your work, co-workers and Desert Oasis Healthcare and Family Hospice Care
- Offer to answer any additional questions that the person, patient or customer may have

Qualifications
- Advanced degree in Nursing, or Advanced Practice Nurse Certification for Nurse Practitioners.
- Current unrestricted license to practice medicine in the State of California. Active DEA number.
- Excellent interpersonal communication (written and verbal) skills.
- Good communication skills, detail oriented, critical thinking and problem-solving skills. Excellent customer service and telephone etiquette skills.
- Valid California driver's license and proof of automobile insurance.
