JOB DESCRIPTION: Case Manager STATUS: Salaried - Exempt
SUPERVISOR: Executive Director HOURS: 35 - 40 hours per week
SALARY RANGE: $41,000 to 46,000 Commensurate with experience

ESSENTIAL FUNCTIONS
The Case Manager develops and implements a plan for guests to move to stable housing along with achieving financial stability and personal success. The incumbent will provide casework, supportive services to families and related data collection along with reporting for guests in all programs. This position requires some evening and weekend hours.
Direct Services
· Interviews with potential guests and maintains waiting list.
· Conducts intakes, including needs assessment and guest history, with new guests.
· Develops individualized housing stabilization and self-sufficiency plans with guests and assists in achieving established goals.
· Help families to identify and utilize community resources.
· Assigns and supervises chores of guests for the Day Center.
· Assists families in their relocation to Congregations.
· Coordinates guest transitions between host congregations and other housing placements as required.
· Computes and monitors income information and financial situation of guests; provide budgeting and money-management education as needed.
· Maintains guest savings records and processes funds in accordance with approved financial procedures and internal controls.
· Meet regularly with guests to discuss progress towards goals, and adjust as necessary.
· Facilitates guest meetings to review program expectations, address concerns, and provide educational support.
· Provides crisis intervention, conflict resolution, case management, and support services for guests.
· Assists with educational programs and holiday activities, including gift distribution
· Monitors guest compliance with program rules and regulations.
· Provides follow-up services to guests who complete the program.
· Updates church host coordinator with face sheet on each family.
· Develops and maintains relationships with landlords, property managers, and housing partners to support guest housing placement efforts.
Advocacy and Referral
· Provides information and referral assistance to all guests.
· Supports guests in advocating for themselves.
· Develops and shares resources for guests/staff.
· Maintains strong professional relationships with referring agencies.
Recordkeeping and Communications
· Maintain accurate and up-to-date case notes and files.
· Submits accurate and timely reports as required.
· Immediately notifies the Executive Director of significant guest safety concerns, policy violations, incidents, or other matters requiring leadership intervention.
· Maintains confidentiality of staff, guests, volunteers and all program participants.
· Represents the program and its constituents, at all times, in a positive manner within the communities served.
Program Operations
· Performs light maintenance and cleaning duties.
· Answer the telephone and directs calls appropriately.
Miscellaneous
· Assists with donations, furniture movement, and maintenance of supply distribution.
· Responds appropriately to emergency situations, including fire alarms, medical incidents, and facility emergencies in accordance with established procedures.
· May be required to use personal vehicle to complete job duties including, but not limited to, donation pick-up, outreach, and off-site meetings.
· Performs all other duties as assigned.

Hours: Full time; salaried - 35 to 40 hours per week; flexible.
Site: 1346 W. Hamilton Street
Other: Primary work location: Day Center and other approved Family Promise locations, off hours support, including host congregations and community partner sites.
Required Qualifications:
· Experience with direct service in a non-profit environment.
- Knowledge of local community resources, agencies and organizations.
· Strong organizational skills, detail-oriented, self-starter, with strong work ethics.
- Ability to foster relationships with diverse faith traditions.
- Excellent communication skills (oral and written).
- Proficient in Microsoft Office applications.
- Ability to occasionally lift up to 50 pounds, navigate stairs, and perform duties in various community
· Experience using case management databases and client tracking systems preferred.
· Criminal Record Check required.
- Child Abuse History Clearance required.
- Demonstrated ability to work effectively with individuals and families from diverse cultural, socioeconomic, and faith background.
- FBI Background Check required.
- Valid Driver’s License and clean driving record required.
- Personal vehicle or access to independent transportation.
- Proof of motor vehicle insurance.

· Bi-lingual (Spanish).
