Job Overview
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team on a PRN (as-needed) basis. In this vital role, you will provide expert physical therapy services to diverse patient populations, including geriatric, pediatric, neurological, orthopedic, inpatient, outpatient, and home health clients. Responsibilities
- Assess patients’ physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to develop tailored treatment plans.
- Implement evidence-based therapy techniques such as trigger point therapy and other physiotherapy modalities to enhance patient outcomes.
- Collaborate closely with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate care plans and discharge planning strategies.
- Document all patient interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards.
- Manage patient rehabilitation programs focused on improving mobility, strength, balance, and functional independence across outpatient clinics, inpatient units, or home health environments.
- Educate patients and their families on treatment procedures, home exercises, and community resources to support ongoing recovery outside clinical settings.
- Monitor progress regularly and adjust treatment plans as needed to ensure optimal recovery trajectories for diverse patient needs.
Requirements
- Valid state licensure or certification as a licensed Physical Therapist with practicing in a physical therapy role.
- Demonstrated experience in outpatient rehabilitation centers, inpatient hospital settings, or home health environments.
- Strong knowledge of physiology, anatomy, medical terminology, orthopedics, neurology, geriatrics, pediatrics, and community care practices.
- Proven ability to develop comprehensive care plans that incorporate discharge planning and patient-centered goals.
- Experience working with geriatric patients and managing complex cases involving mobility limitations or chronic conditions.
- Familiarity with electronic health records (EHR) management systems like EMR systems for accurate documentation of treatments and progress notes.
- Excellent client communication skills with the ability to educate patients effectively about their treatment options and rehabilitation strategies.
- Ability to work independently while collaborating effectively with multidisciplinary teams in outpatient clinics or inpatient hospital settings.
