Job Summary
We are seeking a dedicated and compassionate Case Manager to join our dynamic behavioral healthcare team. In this vital role, you will coordinate client care plans, facilitate communication among clients and various school districts, bridge gaps between clients and their health or social systems, and collaborate with therapists and client support teams to ensure the clients overall wellbeing. Your expertise will help improve client outcomes, optimize resource utilization, and support diverse populations through comprehensive social services and clinical support. Duties
- Interview and observe minor client behavior to identify emotional or developmental struggles
- Develop personalized care plans
- Collaborate with therapist, school nurses and staff, and client families—to coordinate care and facilitate effective communication.
- Ensuring accurate documentation within EHR (Electronic Health Records) systems while maintaining HIPAA compliance.
- Provide supportive guidance, psychoeducation, practical coping and problem solving strategies to assist clients in successfully achieving goals
- Assist patients from diverse cultural backgrounds by providing support when needed and ensuring culturally sensitive care delivery.
- Support discharge planning by coordinating social services, and community-based programs to aid in transitions.
- Maintain detailed documentation of progress notes, treatment plans and interventions to support billing and compliance requirements.
- Engage in ongoing education related to case management services.
Qualifications
- High School Diploma/GED
- Strong background in behavioral/mental health
- Experience in treating adolescents and adults
- Knowledge of ICD coding, CPT coding, ICD-9/10 updates, and utilization review processes.
- Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds.
- Proficiency in EMR/EHR systems along with Microsoft Office applications for documentation and reporting purposes.
- Familiarity with HIPAA regulations, behavioral/mental health terminology, discharge planning procedures, and social services coordination.
