Overview
Join our healthcare team as a Mobile Occupational Therapist, where you will play a vital role in delivering personalized therapy services directly to patients in their homes, clinics, and healthcare facilities. As a key member of our multidisciplinary team, you will utilize your expertise to develop tailored care plans, assist with activities of daily living (ADLs), and support discharge planning to ensure seamless transitions for your patients. Responsibilities
- Conduct comprehensive assessments of patients’ functional abilities, including ADLs, mobility, and cognitive skills across various settings such as home health, outpatient clinics, schools, and skilled nursing facilities.
- Develop and implement individualized care plans that address each patient’s unique needs, goals, and medical conditions.
- Assist patients with activities of daily living such as dressing, grooming, feeding, and toileting to promote independence.
- Collaborate with interdisciplinary teams—including speech therapists, physical therapists, physicians, and case managers—to coordinate care and optimize patient outcomes.
- Support discharge planning by evaluating patient progress and providing recommendations for ongoing therapy or community resources.
- Maintain accurate documentation of patient progress, treatment plans, and case management notes in compliance with healthcare regulations.
Qualifications
- Valid occupational therapy license or certification in the relevant jurisdiction.
- Proven experience working in post-acute care environments including skilled nursing facilities and home health.
- Experience working with diverse populations including geriatrics, individuals with disabilities, and those requiring early intervention services.
- Strong knowledge of physiology, anatomy, medical terminology, and how they relate to occupational therapy interventions.
- Ability to assist with ADLs effectively while promoting patient independence through tailored strategies.
