- Monday-Friday 8-1630, weekends based on department needs
- Conducts new patient interviews and assesses patient's function health and educational needs.
- Monitors patient's lab values, test results and progress.
- Provides patient/family education.
- Coordinates patient discharge needs to ensure smooth transition from hospital to outside agencies
Additional Preferred Requirements
- Masters Degree
- Specialty certification, Certified Heart Failure Nurse or Cardiac Nurse Specialist
- Strong cardiology or ICU background

Northwest HealthCare, six miles west of Christian, offers 24- hour emergency care, the Sleep Disorders Center and a variety of outpatient diagnostic and imaging services.
The Nursing Administration oversees the various Nursing departments and provides clerical support.

Role Purpose
Acts as a patient advocate by providing education, support and interventions throughout the continuum of patient care including pre & post hospitalization. Performs duties by working with physicians to identify and carry out treatment plans.
Responsibilities
- Works with the Medical Director and other physicians to identify and carry out the treatment plan.
- Acts as a Patient Advocate helping to navigate patients through the diagnosis, treatment and rehabilitation process.
- Serves as a resource to assist patients and families, other professionals and the community.
- Provides education for the community and referring physicians.
- Facilitate clinical protocols established by Medical Director(s).
- BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
