Vision Life is seeking an experienced, compassionate, and highly organized Client Case Coordinator to join our growing Supported Living Services (SLS) team. We are looking for a dedicated professional with experience coordinating services for individuals with developmental disabilities and a strong understanding of the SLS model.
Vision Life provides Supported Living Services (SLS) to adults with developmental, intellectual, and behavioral disabilities, empowering them to live independently while promoting dignity, choice, and meaningful community inclusion. The ideal candidate will have experience working within an SLS program, collaborating with Regional Centers, supporting IPP/ISP implementation, coordinating staffing and client services, maintaining compliance with Title 17 requirements, and ensuring accurate, timely documentation.
As a Client Case Coordinator, you will serve as the primary point of contact for assigned clients, families, Direct Support Professionals (DSPs), Regional Center Service Coordinators, and the interdisciplinary team. You will be responsible for coordinating client services, monitoring service delivery, assisting with staffing and scheduling, conducting home visits, completing required documentation, addressing client concerns, and helping ensure each individual receives high-quality, person-centered support. This position requires exceptional organizational skills, the ability to manage multiple priorities, strong communication, and a genuine passion for helping individuals achieve greater independence and an enhanced quality of life.
Responsibilities
Under the direction of the SLS Program Supervisor and Program Director, duties include but are not limited to:
- Coordinate and schedule client services and staff coverage
- Ensure authorized service hours are implemented appropriately
- Conduct intake assessments and participate in client screenings and referrals
- Create IPP and ISP reports independently
- Provide case management services using individualized support approaches
- Mentor, support, and help manage Direct Support Professionals (DSPs)
- Complete monthly, quarterly, and annual reports and documentation
- Participate in home visits and community-based support when necessary
- Provide crisis intervention and behavioral support when needed
- Maintain client files, at-home binders, and updated documentation
- Collaborate with multidisciplinary teams and community resources
- Coordinate with Regional Centers, vendors, behavioral teams, and outside professionals
- Participate in on-call support rotation in accordance with company policy
- Deliver exceptional customer service and client-centered care
Qualifications
- High school diploma/GED with relevant experience required
- Associate’s or Bachelor’s degree in Human Services, Psychology, Social Work, or related field preferred
- Experience working with individuals with developmental disabilities, intellectual disabilities, and behavioral disabilities preferred
- Knowledge of Regional Center and DDS protocols is a plus
- Strong communication, organizational, and critical thinking skills
- Ability to manage multiple tasks independently and professionally
- Valid California Driver’s License required
- Reliable transportation and current auto insurance required
- DOJ/Fingerprint clearance required
- CPR/First Aid certification preferred
- CPI certification is a plus
- Bilingual or ASL skills are a plus
Schedule
- Part Time
- Monday–Wednesday
- Flexible availability may be required based on client needs

Travel Requirements
This position requires travel throughout the Pasadena and surrounding Los Angeles County areas to support clients in community and residential settings. Mileage reimbursement is provided.
