What You'll Do
T
he Case Manager is responsible for
developing and monitoring comprehensive participant service plans in collaboration with assigned participants. This includes, but is not necessarily limited to, internal and external referrals for services to stabilize medical and mental health conditions, barrier removal, job readiness coaching, job placement assistance, and retention services.
Your responsibilities will include:
Initiate services for participants following their clinical assessment or clinical reassessment, providing orientation to services, explaining their rights and responsibilities, and answering questions.
Regularly meet with assigned participants to monitor progress and compliance with wellness plans, as well as to assist with mitigating barriers to employment. These incudes, but is certainly not limited to housing, domestic violence, childcare, transportation, and reasonable accommodations.
Case conference with other
WeCARE
service providers on an as needed basis, including vocational evaluators, work readiness and computer instructors, job placement coordinators, job preparation counselors and WEP specialists.
You're a great fit for this role if you have:
Bachelor’s degree in social work or related field, such as psychology, vocational rehabilitation, or occupational rehabilitation. MSW or master’s degree in a related field preferred.
Compensation
$23.08-$24.04/hr.
