What You’ll Do:
Evaluate and develop individualized treatment plans for residents in a SNF/LTC environment
Deliver high-quality, evidence-based physical therapy services
Drive functional outcomes that restore independence and confidence
Collaborate closely with nursing, therapy, and interdisciplinary teams
Document clearly and efficiently using Net Health and PCC systems
Mentor and support PTA team members as needed
What We’re Looking For:
Active Pennsylvania Physical Therapist license
Experience in Skilled Nursing or Long-Term Care preferred
Strong clinical judgment and decision-making skills
Confidence leading patient care and collaborating with a team
A positive, motivated, and solution-oriented mindset
