What You’ll Do:
- Deliver one-on-one speech, language, and swallowing therapy in the home
- Perform diagnostic assessments and craft customized treatment plans
- Provide functional training tailored to communication, cognition, and feeding
- Educate clients and families on strategies that reinforce daily progress
- Document care using EMR and collaborate with the care team to ensure continuity
Qualifications:
- Active Speech Language Pathologist (SLP) license in Minnesota
- Valid driver’s license and reliable vehicle for travel
- EMR experience preferred
- Strong communication skills and a commitment to patient-centered care
