A Brief Overview
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.

Education

Master's Degree from an accredited school of Social Work (Required)
Work Experience

Hospital/Health Agency experience. (Preferred)

Position Details
This position will attend to activations, addresses discharges and assessment needs in the TSICU, offers psychosocial support, and also supports discharge of floor patients as needed. The schedule is Mon-Fri, 3P-11P shift, but occasional weekends and holiday coverage is required.
