We are seeking a compassionate, organized, and clinically skilled Charge Nurse to join our nursing team and provide exceptional care to our residents. This is a full-time weekday position and is also part of our weekend on call rotation. This position offers an excellent opportunity for a dedicated nurse who enjoys leadership, collaboration, and delivering high-quality resident-centered care.
Position Summary
The Charge Nurse is responsible for directing the daily operations of the nursing unit while ensuring residents receive safe, compassionate, and high-quality care. The Charge Nurse serves as a clinical leader by supervising nursing staff, coordinating resident care, communicating with physicians and families, and maintaining compliance with all regulatory and facility standards.
Responsibilities
- Conduct job responsibilities in accordance with the FCRC Code of Conduct, policies and procedures, applicable federal and state laws, and professional standards.
- Promote adherence to FCRC's Code of Business Conduct by monitoring employee performance and identifying and responding to compliance issues.
- Ensure resident safety by providing and directing nursing care to attain or maintain each resident's highest level of physical, mental, and psychosocial well-being according to individualized care plans.
- Ensure residents remain free from abuse and neglect.
- Assess, observe, and evaluate resident conditions; communicate directly with physicians and/or the Medical Director; implement physician orders; and communicate order changes to nursing staff.
- Identify restorative and rehabilitation needs for long-term care residents.
- Communicate with residents and families regarding plans of care, changes in condition, and provide education when appropriate.
- Participate in interdisciplinary meetings, family care conferences, and discharge planning meetings.
- Evaluate and update resident care plans to accurately reflect current conditions in collaboration with the MDS Coordinator.
- Complete resident admissions and readmissions, including medication reconciliation, physician orders, EMAR and ETAR setup, initial assessments, care plans, and coordination with pharmacy.
- Coordinate resident discharges, including medication reconciliation, discharge education, prescriptions, transportation, equipment, referrals, and communication with primary care providers.
- Demonstrate competency in resident assessment, IV therapy, phlebotomy, enteral feeding, wound care, and other skilled nursing treatments.
- Follow facility policies regarding resident death and provide compassionate support to families.
- Complete all required nursing documentation to meet regulatory standards, including monitoring weights, vital signs, intake and output, bowel records, blood glucose levels, and INRs.
- Schedule required nursing assessments, physician visits, and resident appointments.
- Lead nursing shift report at the beginning and end of each shift, ensuring effective communication of new orders, treatments, follow-up needs, and outstanding issues.
- Participate in risk management, quality improvement initiatives, and management meetings as requested.
- Perform resident and environmental rounds to monitor quality of care, infection prevention, safety, and cleanliness.
- Complete resident incident reports, infection reports, and ensure appropriate follow-up.
- Direct the daily operations of the nursing unit, including staff supervision, scheduling, performance evaluations, orientation of new employees, staffing coordination, coaching, and disciplinary action when appropriate.
- Assist staff nurses and LNAs with resident care as needed.
- Respond effectively to clinical emergencies, administer CPR.
- Participate in the Administrative RN on-call rotation.
- Perform other duties as assigned.
Qualifications
- Current RN with at least 2 years experience.
- Long-term care or skilled nursing experience preferred.
- Strong assessment, clinical, leadership, and communication skills.
- Ability to supervise, mentor, and support nursing staff.
- Knowledge of state and federal long-term care regulations.
- Current CPR certification.

Pay: $45.00 - $50.00 per hour
