Position Summary
The Nurse Manager serves as the clinical leader for the Adult Day Center, ensuring the delivery of high-quality nursing care while supporting the daily operations of the center. This role is designed as a leadership development position with the expectation that the successful candidate will transition into a floating the Day Center Director role upon demonstrating proficiency in program operations, regulatory compliance, financial management, and team leadership.The ideal candidate is a compassionate, organized, and visionary nurse who is passionate about caring for older adults and individuals with chronic medical conditions while developing the skills necessary to lead an interdisciplinary adult day program.
Essential Responsibilities
Clinical Leadership
- Provide oversight and supervision of all nursing services within the Adult Day Center's health program.
- Conduct participant nursing assessments, reassessments, and individualized care planning.
- Monitor participants' medical conditions and coordinate interventions as needed.
- Administer or oversee medication administration and treatments according to physician orders and organizational policies.
- Collaborate with physicians, therapists, social workers, dietitians, and other interdisciplinary team members.
- Ensure compliance with infection prevention, medication management, and clinical documentation standards.
- Respond appropriately to participant emergencies and oversee emergency preparedness procedures.
Staff Leadership
- Supervise certified nursing assistants and/or medication technicians.
- Foster a collaborative, respectful, and participant-centered culture.
- Provide health education to participants, families, and staff as needed
Quality & Compliance
- Ensure compliance with all federal, state, and local Adult Day Health regulations.
- Participate in quality assurance and quality improvement initiatives.
- Participate in surveys, audits, and accreditation activities.
- Maintain accurate clinical documentation and regulatory records.
Operational Leadership (Director Development)As part of the leadership succession plan, the Nurse Manager will progressively assume responsibilities including:
- Daily center operations
- Budget planning and fiscal oversight
- Census growth and participant retention
- Community outreach and referral development

- Vendor and contract management

Transition to Day Center DirectorUpon successful completion of the leadership development program and demonstration of competency, the Nurse Manager will transition into the role of floating Adult Day Center Director. Responsibilities will expand to include overall operational, financial, and administrative oversight of the Adult Day Health Center while maintaining excellence in participant care and regulatory compliance.QualificationsRequired
- Active Registered Nurse (RN) license in good standing.
- Bachelor's degree in Nursing (BSN) or higher.
- Minimum of three years of nursing leadership experience.
- Experience working with older adults, geriatrics, chronic disease management, rehabilitation, or long-term care.
- Strong knowledge of state and federal healthcare regulations.
- Excellent communication, leadership, and organizational skills.
Preferred
- Experience in Adult Day Health, PACE, home health, geriatrics, or skilled nursing.
- Experience managing multidisciplinary teams.
- Budgeting and operational management experience.
- Quality improvement and regulatory survey experience.
