Nurse PractitionerWe are seeking an experienced, independent, highly skilled, and technology-proficient Nurse Practitioner (NP) to join our primary care team in Pilot Point. The ideal candidate will provide comprehensive, patient-centered primary care with a strong emphasis on preventive medicine, while also playing an active role in community outreach, patient growth, and clinical leadership.The NP will have an opportunity to help establish and grow a new primary care practice, build relationships within the local community, and contribute to the development of clinical workflows and programs.The role will leverage advanced healthcare technology, including Clinical Decision Support Systems (CDSS), Remote Patient Monitoring (RPM), telehealth, and AI-powered clinical documentation tools. Training will be provided for all clinical systems and digital tools.In Texas, the NP will practice under a delegation agreement with a supervising physician, in compliance with applicable state regulations.Key ResponsibilitiesComprehensive Primary CareProvide comprehensive primary care services, including:

- Develop personalized preventive care plans.

Community Outreach & Patient Growth
- Represent the clinic within the local community and develop relationships with:

- Participate in community health events, screenings, educational programs, and outreach activities.
- Help increase awareness of clinic services and improve access to primary care.
- Identify opportunities to generate new patient referrals and strengthen community partnerships.
- Work with the management and marketing teams to support patient acquisition and retention initiatives.
- Serve as a clinical ambassador for the practice within the community.
Clinical Leadership
- Help establish and improve clinical workflows as the practice grows.
- Support development and implementation of clinical protocols and SOPs.
- Mentor and guide clinical staff as the team expands.
- Promote a culture of teamwork, accountability, patient safety, and continuous improvement.
- Collaborate with physicians and multidisciplinary teams to ensure high-quality care.
- Take an active leadership role in building a high-performing primary care practice.

Qualifications & Skills
- MSN or DNP from an accredited institution.
- Board-certified Nurse Practitioner — Family Medicine or related specialty.
- Active Texas NP license or eligibility to obtain.
- Ability to practice under a delegation agreement with a supervising physician.
- BLS required; ACLS preferred.
- Fluent in Spanish strongly preferred, depending on patient population.
- Strong primary care and preventive care experience.
- Experience with community outreach, health education, or referral development strongly preferred.
- Demonstrated leadership or team-lead experience preferred.
- Proficiency with EHR systems.
- Familiarity with CDSS and AI-assisted documentation preferred.
- Experience with telemedicine, RPM, or CCM preferred.
- Excellent communication and interpersonal skills.
- Strong clinical judgment and decision-making ability.
- Strong commitment to patient-centered and preventive care.
- Ability to work independently while collaborating effectively with physicians and staff.
- Comfortable working in a growing practice where flexibility and a willingness to take on multiple responsibilities are important.
