Community Home Health Care specializes in providing Private Duty Nursing for both Pediatric and Adult clients in the Capital District and surrounding counties. We are the most experienced and knowledgeable PDN Licensed Home Care Service Agency in the area.
We are looking for an RN or LPN to staff a case in Cropseyville, NY.
1-year-old male patient. The patient has a trach, is on a vent, has a feeding tube and an ostomy. This patient requires a nurse who is confident with vent and trach care.
Hours:
- Friday - Sunday: Overnights -- must have strong vent/trach skills
- Saturday & Sunday: Days
Responsibilities
- Provide direct patient care in home settings, ensuring comfort and safety.
- Administer medications and treatments as prescribed, including IV infusions and tube feeding, if applicable.
- Conduct patient assessments and monitor vital signs, documenting findings in EHR systems.
- Assist with airway management as needed.
- Educate patients and families on care plans and health management.
- Uphold HIPAA regulations and maintain confidentiality in all interactions.
**40-45/hr rate is for those that can commit to 32+ hours per week.
