Responsibilities
- Deliver comprehensive nursing care to residents, including medication administration, vital sign monitoring, and physical assessments.
- Assist with specialized memory care procedures such as behavioral health management and dementia-specific interventions.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with residents’ needs.
- Manage IV infusions, catheter care, tube feeding, and other complex medical procedures with precision and compassion.
- Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
- Conduct triage assessments for urgent health concerns, providing timely interventions or referrals as necessary.
- Support discharge planning and coordinate with families and healthcare providers for seamless post-acute or hospice care transitions.
Skills
- Proven experience in assisted living, long-term care, or memory care environments with a focus on Alzheimer’s and dementia care.
- Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
- Proficiency in EMR/EHR systems such as Epic, Athenahealth, or eClinicalWorks; familiarity with ICD coding (ICD-9/10) is highly desirable.
- Ability to perform complex procedures including IV insertion, catheterization, phlebotomy, and sterile technique.
- Excellent patient assessment skills coupled with acute pain management and infection control expertise.
- Critical thinking skills for triage situations involving urgent or emergent health issues in senior populations.

Pay: $29.00 - $31.00 per hour
