Overview
We are seeking a dedicated and experienced Hospice RN Case Manager to join our compassionate team with an expectation in excellence and customer service. The ideal candidate will possess extensive clinical expertise, including critical care, emergency medicine, and post-acute care experience. As a Hospice RN Case Manager, you will coordinate comprehensive patient care plans, ensuring comfort and dignity for individuals facing end-of-life journeys. This role offers an opportunity to make a meaningful difference through skilled nursing and case management within a supportive environment.
Duties
Attendance
Excellent Customer Service
· Provides skilled nursing care under the physician’s orders
· Provides nursing assessment, intervention, and evaluation for each individual client
· Promotes, maintains, and restores health through teaching, counseling and use of appropriate preventative, rehabilitative and /or palliative measures
· Collaborates with the client’s physician regarding the plan of care and arranges for the Face to Face as required.
· Maintains accurate documentation in timely manner
· Participates in case conferences, or IDG meetings
· Case manages home care/hospice needs of clients to provide a safe care plan, using all available community resources
· Delegates personal care or homemaking duties to home health aides and homemakers. Participates in instruction, supervision and evaluation of aides and homemakers
· Assures quality of care is maintained and client’s needs are appropriately met
· Assists with Quality Assurance and Review as requested
· Demonstrates alignment with the Agencies mission, vision, and values
· Attends staff meetings and educational programs as required
· Follows State and Federal Regulations for Home Care and Hospice
· Follows all safety policies and procedures
· Other duties as assigned
Qualification
· Registered Nurse licensed with State of Minnesota
· At least one year Medical/Surgical or related nursing experience
· Hospice or Home Care Experience Prefered
Pay: $42.00 - $47.00 per hour
