General
Assist individuals eligible under the state plan in gaining access to needed recovery supports, medical, social, educational, and other services.
Duties include:

assessing the individualized needs of an individual

coordinating the provision of services

documenting services and outcomes.
Qualifications and Education Requirements

Bachelor’s degree in social work or the human services field
-
Must have a current driver’s license
