Job Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have experience in Hospice counseling and working with elderly individuals with terminal disabilities. This role involves providing support and guidance to clients in various settings, including outpatient services and special education environments. The Social Worker will play a crucial role in enhancing the well-being of clients through effective hospice and palliative interventions.
Responsibilities
- Conduct assessments to determine clients' needs and develop individualized treatment plans.
- Provide family counseling to support families in navigating challenges.
- Facilitate sessions to promote social skills and emotional well-being.
- Implement behavioral therapy techniques tailored to each client's unique situation.
- Utilize motivational interviewing strategies to encourage client engagement.
- Maintain accurate medical documentation of client interactions and treatment outcomes.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Advocate for clients' needs within the community and coordinate additional resources as necessary.
Qualifications
- A Master's degree in Social Work (MSW) from an accredited institution is required.
- Experience in Hospice counseling, working with individuals with terminal diagnosis.
- Familiarity with outpatient services and hospice and palliative care methodologies is advantageous.
- Strong skills in behavioral therapy and motivational interviewing techniques are essential.
- Excellent communication skills, both verbal and written, for effective documentation and client interaction.
- Ability to work collaboratively within a team-oriented environment while maintaining client confidentiality.
