Healthy Connections’ mission is to enrich the lives of people with developmental disabilities by providing them a safe and inclusive environment, either in their own home or our facility, that fosters life skill - social, personal hygiene and independence.
We’ve served the Columbus, Ohio, and surrounding areas since 2006. We offer a variety of long-term sustainable programs that enrich the lives of people with developmental disabilities – with offerings that include nursing, home care, adult day care and summer camps. Home Health Aide – Job Description for Posting Sites
Aides follow a physician established treatment plan and work as part of the client’s healthcare team Coordinated by their RN Case Manager. The type of care may vary by the client’s needs however are generally tasks to assist with daily living, such as: feeding, dressing, bathing - or can be more “hands-off” such as running errands, housekeeping, etc.

We are seeking a compassionate Home Health Aide (LPN/RN needs tweaking for those) to provide assistance in daily living and personal care services in clients’ homes in accordance with an established care plan. This position is not only rewarding professionally, but also personally.

Responsibilities
Provide health care services in patients residences
Perform domestic and household tasks
Transport and accompany patients to doctors office or to hospital
Administer simple prescribed medications
Assist with client’s personal care activities
Monitor patients (vital signs, temperature, respiration, etc.) and report on their condition
Maintain patients care records and document provided services
Assist patients with mobility and physical therapies/exercises
Instruct and counsel patients and families on diet and exercise
Collect routine specimens
Provide companionship and basic emotional or psychological support
Skills
Prior work experience as home health aide
Ability to monitor vital signs and to collect specimens
Familiarity with basic nutrition and personal hygiene standards
Nursing and health care administration skills
Caring and compassionate personality
Current CPR certificate
Proficiency in English
Working locations: Our office and adult day care center is located in Westerville, Ohio. However, we treat patients of all ages and conditions throughout the Columbus area within their own homes.
