Physical Therapist (PT) – Long-Term Care / Skilled Nursing Facility

Senior Care of Marion is seeking a dedicated and compassionate Physical Therapist (PT) to join our interdisciplinary team in a long-term care and skilled nursing setting. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide skilled therapy services designed to improve mobility, independence, safety, and quality of life.

Essential Responsibilities
- Perform comprehensive physical therapy evaluations and assessments.
- Develop, implement, and modify individualized treatment plans based on resident needs and goals.
- Provide skilled therapeutic interventions including gait training, balance training, strengthening, mobility training, pain management, and fall prevention strategies.
- Supervise and direct Physical Therapist Assistants (PTAs) and supportive personnel in accordance with state and federal regulations.
- Monitor resident progress and adjust treatment plans as appropriate.
- Complete accurate and timely documentation, including evaluations, progress notes, recertifications, and discharge summaries.
- Educate residents, families, and caregivers regarding treatment plans, safety precautions, and home exercise programs.
- Collaborate with physicians, nursing staff, rehabilitation professionals, and other members of the interdisciplinary team.
- Participate in care plan meetings and discharge planning activities.
- Ensure compliance with all facility policies, professional standards, and regulatory requirements.
Qualifications
Required:
- Degree in Physical Therapy from an accredited program.
- Current, active Physical Therapist license in the State of [State].
- Current CPR certification or willingness to obtain upon hire.
- Strong clinical assessment and treatment skills.
- Excellent communication, organizational, and interpersonal skills.
Preferred:
- Experience in long-term care, skilled nursing, subacute rehabilitation, or geriatric therapy.
- Experience with electronic medical records (EMR) systems.
- Knowledge of Medicare Part A and Part B rehabilitation services.

- Ability to stand, walk, bend, lift, and assist residents throughout the workday.
- Ability to transfer and reposition residents safely.
- Ability to lift up to 50 pounds with or without reasonable accommodation
Experience:
- Long term acute care: 1 year (Required)
- Assisting with ADLs: 1 year (Required)
License/Certification:
