Responsibilities
- Deliver comprehensive home health care assistance by supporting patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility.
- Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; promptly report any concerns or changes to supervising healthcare professionals.
- Assist with medication administration following prescribed instructions while maintaining accurate medical documentation in electronic health record (EHR) systems.
- Provide specialized care for patients with dementia, Alzheimer’s disease, or other cognitive impairments through memory care techniques and compassionate engagement.
- Safely transfer patients using proper patient transfer methods and equipment such as Hoyer lifts or gait belts; ensure patient safety during movement and positioning.
- Maintain patient confidentiality and adhere to all home healthcare regulations and HIPAA guidelines while managing sensitive medical records and documentation.
- Support end-of-life care needs with dignity and respect, collaborating with hospice teams to ensure comfort and emotional support for patients and families.
Qualifications
- Valid certification as a Home Health Aide (CHHA) issued by the state licensing authority.
- Proven experience working with geriatric patients, individuals with disabilities, or in assisted living environments.
- Familiarity with EMR/EHR systems for documentation and patient record management.
- Knowledge of medical terminology, catheter care, wound management, and basic life support (BLS) including CPR certification.
- Ability to perform heavy lifting when necessary and utilize patient transfer equipment safely; physically capable of standing, walking, bending, and performing caregiving tasks for extended periods.
- Strong communication skills for effective client interaction and collaboration with healthcare teams.
- Understanding of home health care regulations, HIPAA compliance, and infection control procedures.
Pay: $50.00 - $60.00 per hour
