Position Available: Physical Therapist (PT)
Status: Part-Time or PRN
Areas to serve: Weber County, Utah
Job Duties and Responsibilities:
- Provide direct physical therapy services to patients, including therapeutic exercises, manual therapy, and modalities, all in the comfort of their homes.
- Develop and implement individualized treatment plans based on patient assessments and rehabilitation goals.
- Educate patients and their families on exercises and activities to enhance recovery, mobility, strength, and overall physical function.
- Monitor and document patient progress, adjusting treatment plans as necessary to ensure optimal outcomes.
- Collaborate with healthcare providers, caregivers, and interdisciplinary team members to ensure comprehensive care for patients.
- Maintain accurate and up-to-date patient records, documenting treatment sessions, progress notes, and any changes in condition.
- Ensure compliance with regulatory standards and best practices in physical therapy.
- Provide guidance and supervision to Physical Therapist Assistants (PTAs) as needed.

Qualifications:
- Doctorate or Master's degree in Physical Therapy from an accredited program.
- Current state licensure as a Physical Therapist (PT).
- Previous experience in home health or hospice care is advantageous.
- Strong organizational and time management skills with the ability to prioritize tasks and manage multiple responsibilities simultaneously.
- Excellent communication and interpersonal skills with the ability to interact effectively with patients, families, and healthcare professionals.
- Proficiency in electronic health records (EHR) systems and Microsoft Office suite.
- Compassionate and empathetic attitude towards patients and their families.
- Ability to work independently and as part of a collaborative team.

Pay: $60.00 - $110.00 per hour
