Overview
We are seeking a dedicated and compassionate Hospice Aide/CNA to join our team. In this role, you will deliver vital personal care and support services to patients receiving hospice care, ensuring comfort, dignity, and respect. The ideal candidate will have experience in patient observation, assisting with activities of daily living (ADLs), and working within home health or long-term care environments. Your efforts will directly contribute to improving the quality of life for individuals facing serious illnesses.
Responsibilities
- Provide compassionate assistance with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts.
- Monitor and document patient condition accurately, including vital signs, medication administration, and changes in health status using electronic health record (EHR) systems.
- Assist with medical procedures including catheter care, wound care, and other hospice-specific treatments under the supervision of nursing staff.
- Support patients with memory care needs related to Alzheimer’s disease and dementia by implementing specialized care plans.
- Maintain a clean, safe environment by adhering to infection control protocols and performing light housekeeping tasks such as laundry and meal preparation.
- Observe and report any significant changes in patient condition promptly to the healthcare team to ensure timely intervention.
- Uphold patient confidentiality in compliance with HIPAA regulations while managing medical records and documentation.
Requirements
- Valid CNA certification or equivalent licensure required; hospice or home health experience preferred.
- Demonstrated experience working with individuals with disabilities, developmental disabilities, or in senior care settings.
- Knowledge of medical terminology relevant to hospice care and long-term caregiving practices.
- Certification in CPR is required; additional training in dementia or Alzheimer’s care is advantageous.
- Proficiency in EMR/EHR systems for documentation; familiarity with medication administration protocols.
- Ability to work independently in a home setting while maintaining professional standards of patient care.

Pay: $18.00 - $24.00 per hour
