Responsibilities:
- Provide case management services to patients, ensuring their needs are met and their care is coordinated effectively via phone from our Solon, OH office.
- Utilize knowledge of physiology and patient care to assess and monitor patients' conditions.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Conduct discharge planning to ensure smooth transitions for patients leaving the hospital.
- Provide critical care support to patients in need.
- Maintain accurate and up-to-date patient records.
Qualifications:
- Must be a certified Medical Assistant or STNA with a valid license.
- Previous experience in managed care or case management is preferred.
- Strong knowledge of physiology and patient care practices.
- Excellent communication and interpersonal skills.
- Proficient in computers and EMR knowledge.
- Familiarity with inhale medications and their administration.
