Women's Health Registered Nurse (RN)
Position Summary
Norwalk Community Health Center is seeking a compassionate and dedicated Women's Health Registered Nurse (RN) to join our multidisciplinary care team. The Women's Health RN plays a vital role in supporting providers, coordinating patient care, and improving health outcomes for women throughout pregnancy, postpartum, and routine gynecologic care. This position serves as a patient advocate, educator, and care coordinator while providing direct clinical nursing support in a fast-paced outpatient setting.
Schedule: Full-Time or Part-Time, Monday-Friday, 8:00 AM-5:00 PM
Key Responsibilities
- Execute nursing care under the direction of the provider, including assessments, triage, medication administration, venipuncture, injections, vaccines, and procedure support.
- Participate in code blue calls and other patient emergency responses as needed.
- Coordinate prenatal, postpartum, women’s health, chronic disease, and transition-of-care needs, including referrals, diagnostic testing, durable medical equipment orders, hospital/ER follow-up, and care management.
- Educate patients on pregnancy, postpartum care, infant feeding, chronic disease management, medications, preventive health, and available community resources.
- Communicate lab results, treatment plans, medication changes, and follow-up instructions under provider direction and established protocols.
- Maintain accurate, timely documentation in the electronic medical record.
- Track and follow up with women’s health and perinatal patients, including postpartum outreach, missed appointments, high-risk transitions, hospital admissions, as required.
- Support care team operations by collaborating with providers, clinical support staff, Population Health, Chronic Care Coordinators, and community partners.
- Participate in quality improvement, data tracking, staff training, competency verification, and clinic/care team meetings.
- Maintain emergency equipment, clinic medications, supplies, and required logs to support safe patient care.
Qualifications
- Current Registered Nurse (RN) license in the State of Connecticut.
- Current CPR/BLS Certification.
- Clinical nursing experience preferred, ideally in women's health, OB/GYN, primary care, ambulatory care, or acute care.
- Strong assessment, triage, patient education, and care coordination skills.
- Excellent communication and interpersonal skills with a commitment to providing compassionate, culturally sensitive care.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Bilingual (English/Spanish) preferred but not required.
- Ability to work onsite Monday through Friday from 8:00 AM to 5:00 PM.
- Current immunizations and TB screening as required by organizational policy.
