SUMMARY
The peer specialist uses experiences with personal recovery to assist and support ACT clients by offering hope and reassurance, provides peer interventions regarding symptoms of mental illness engaging clients through outreach support in client’s home and community and relapse prevention.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Participates in the ongoing assessment and treatment engagement of ACT clients.
- Completes, as directed, elements of the comprehensive assessment for clients within 60 days of admission and coordinates information with team to effectively plan treatment.
- Provides direct service, which includes teaching daily living skills in medically necessary services such as grocery shopping, maintaining clean housing, using public transportation, maintaining self-care and exploring employment opportunities.
- Transports and/or accompanies clients to appointments related to community services and resources.
- Locates and/or develops self-help and support groups for clients, families and significant others.
- Attends ACT Team meetings to create an integrated, multidisciplinary team approach to services: ~ Treatment Planning Meeting- weekly or as needed to develop and review treatment plans/decisions
- ~ Clinical Supervision Meeting- 1-2 times monthly for training, case consultation and professional development of ACT staff
- ~ Daily Team Meetings- to organize work for each day and review recent contacts with each client.
- Maintains compliance with all applicable practice standards and guidelines
- Maintains client confidentiality and adherence to HIPAA requirements
- Completes all required documentation in a timely manner consistent with agency guidelines.
- Maintains agency productivity standards
- With documented consent, engages client’s family and/or significant others to provide education regarding mental illness and community resources
MARGINAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Works in close collaboration with physicians and other health care personnel in recipient evaluation and treatment to further their understanding of significant social and emotional factors underlying recipient’s problem. Assist clients in developing self directed care/ advance directive plans
- Promotes recovery through:~ Providing encouragement

- ~ Role modeling and sharing experiences
- Provides training and education to other ACT members and clients on:~ Advocacy Organizations and Community Resources~ Psychiatric Advance Directives
- ~ Peer Support
- ~ Recovery Model
- Provides direct service, including social and emotional support
- Assist clients creating goals and developing the skills and resources to achieve those goals

Attends ACT Team meetings to create an integrated, multidisciplinary team approach to services:

Provides On- Call crisis response and intervention services
Maintains compliance with all applicable practice standards and guidelines
Maintains client confidentiality and adherence to HIPAA requirement
Helps recipient and family to understand, accept, and follow substance abuse recommendations.
Maintains a professional relationship with staff, peers, and upper management.
Adheres to the policies, procedures, and work rules set by the Agency.
QUALIFICATIONS
- The Peer Support Specialist brings a unique perspective to ACT because of his/her own experiences with serious and persistent mental illness.
- The ACT Peer Support Specialist shall have documented training in the following areas:~ Peer support~ Psychiatric advance directives
- ~ Advocacy Organizations
- ~ Recovery Model

- Maintain active state drivers license/identification and insurance if driving
- MVR background check
- Criminal background check
- TB test
EDUCATION and/or EXPERIENCE
The ACT Peer Support Specialist shall have documented history and competency in the following areas, or receive core training to be completed within six months of hire:
~ Engagement and motivational strategies to work with dually diagnosed clients
~ Experience and training in relapse prevention
