Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications via injections, IV infusions, catheterizations, and other methods following strict protocols and safety standards.
- Support acute pain management strategies and assist with complex medical procedures such as tube feeding, sterile processing, and wound suturing.
- Monitor patients’ conditions continuously, including airway management with ventilators or oxygen therapy, and respond promptly to emergent situations like acute distress or trauma.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients with diverse needs such as dementia care, Alzheimer's disease, behavioral health issues, or infectious diseases.
- Maintain accurate medical documentation in compliance with HIPAA regulations and ensure all records are up-to-date within EMR/EHR systems.
- Educate patients and their families on health management topics including medication administration, discharge planning, nutrition, and caregiving techniques.
Experience
- Valid state licensure as a Licensed Practical Nurse (LPN) with current CPR/BLS certification.
- Demonstrated experience working with geriatric populations in settings such as skilled nursing facilities or long-term care environments.
- Proven background in acute care settings such as hospitals or emergency rooms; ICU or trauma center experience is highly valued.
- Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and electronic health records (EMR/EHR) systems like Epic or Cerner.
- Experience providing specialized care including IV infusion therapy, hospice care, pediatric or neonatal nursing (including neonatal ventilator management), or behavioral health support is preferred.
- Knowledge of infection control procedures and long-term care regulations to ensure compliance and safety standards are met.
- Ability to perform physical examinations, specimen collection/processing, venipuncture, phlebotomy, catheter care, and first aid techniques confidently.
