The Community EMS Worker provides community-based health support services through in-home patient visits focused on chronic disease management, wellness checks, fall prevention, and home safety evaluations.
Typical shifts include approximately 7–8 patient visits, as determined by Taunton Human Services leadership.
This position is 8 hours/week (part time) on Mondays.
- Complete daily vehicle checklist in Traumasoft
- Sign on/off with the Medical Call Center at the start and end of shift
- Arrive to first scheduled patient visit by 8:00 AM
- Obtain and follow daily patient visit schedule
- Notify MCC and Taunton Housing Authority of any delays
- Report supply needs to MIH Supervisor
- Document all visits in the Taunton Human Services platform
- Participate in team meetings as needed
- Health promotion screenings
- Chronic disease support with focus on:
- Cancer
- Heart disease
- Diabetes
- Basic vital signs:
- Blood pressure
- Heart rate
- Respiratory rate
- Oxygen saturation
- Weight
- Blood glucose testing via finger stick
- Basic skin assessments
- Home and community fall prevention evaluations
- Home safety evaluations:
- Smoke detectors
- Carbon monoxide detectors
- Accessibility concerns
- Nutritional access assessments
- Medication instruction review (no dispensing or pill counting)
- Activation of emergency services when appropriate
- Single rescuer CPR per EMD instructions
- Massachusetts EMT certification
- Minimum 3 years EMT experience
- At least 1 year of 911 experience
- Healthcare Provider CPR certification
- Valid unrestricted Massachusetts Driver’s License
- Strong verbal and written communication skills
