Responsibilities
- Dispense medications accurately while adhering to all legal and safety standards, including aseptic techniques for sterile preparations and compounding medications.
- Conduct thorough patient assessments to determine appropriate medication therapy, considering physiology, medical history, and current health status.
- Provide expert consultation on medication management, pain management, immunizations, and medical terminology to healthcare teams and patients alike.
- Manage inventory levels efficiently, ensuring proper storage, stock rotation, and compliance with HIPAA regulations for patient privacy.
- Utilize pharmacy software and electronic health record (EHR) systems to document prescriptions, medication histories, and patient interactions accurately.
- Administer immunizations and perform IV insertions or infusions with precision using aseptic technique in accordance with hospital protocols.
- Collaborate with medical staff on complex cases involving pediatrics, critical care, or acute care settings to develop tailored treatment plans.
- Educate patients on medication administration, potential side effects, and adherence strategies to promote optimal health outcomes.
- Participate in medical management initiatives such as medication reconciliation and review of medication therapy for appropriateness.
Requirements
- Valid pharmacist license with current registration in the applicable jurisdiction.
- Proven experience in pharmacy practice within hospital or acute care settings; LTC pharmacy experience is highly desirable.
- Strong clinical pharmacy background with knowledge of physiology, anatomy, and medical terminology.
- Experience with pharmacy software systems and EMR/EHR platforms for documentation and communication purposes.
- Demonstrated competence in compounding medications and aseptic techniques for sterile preparations.
- Familiarity with inventory management practices and medication administration protocols.
- Critical care experience or residency training emphasizing patient assessment and medical management is preferred.
- Knowledge of immunizations procedures, IV infusion techniques, and pain management strategies.
- Excellent communication skills to effectively collaborate with healthcare teams and educate patients.
