Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote independence and dignity.
- Monitor patients’ health status by observing vital signs such as blood pressure, pulse, respiration, and temperature; report changes to healthcare professionals promptly.
- Administer medications accurately following care plans while maintaining compliance with HIPAA regulations to protect patient confidentiality.
- Support clients with mobility using proper patient transfer methods and equipment like Hoyer lifts or manual handling techniques to ensure safety during movement.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking to promote healthy living.
- Provide companionship and emotional support, fostering positive communication and engagement with clients and their families.
- Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces.
- Assist with medical procedures such as wound care or hospice support as directed by healthcare providers.
- Support clients in community outings or transportation needs using personal transportation assistance or mobility aids.
- Document all care activities accurately in client records while adhering to home health standards and regulations like HIPAA.
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Demonstrated experience in patient observation, medication administration, and patient monitoring is essential.
- Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, and neurodevelopmental disorders enhances your ability to provide specialized support.
- Hands-on experience with assistive devices such as Hoyer lifts or patient transfer methods is advantageous.
- Familiarity with developing or following care plans tailored to individual needs is important for delivering consistent quality care.
- Experience working with individuals with developmental disabilities or intellectual disabilities supports diverse client needs.
- Certification in CPR and First Aid is required; additional certifications like HIPAA training or nursing assistant credentials are a plus.
- Strong communication skills for effective client interaction and collaboration with healthcare teams are essential for success in this role.
