Overview
We are seeking an experienced and compassionate Designated Coordinator (DC) to oversee the delivery of high-quality services in a 245D Community Residential Setting. The ideal candidate will have experience supporting individuals with mental health diagnoses, co-occurring disorders, and substance use challenges, while ensuring compliance with Minnesota 245D regulations.
The Designated Coordinator will be responsible for coordinating services, conducting assessments, developing support plans, supervising direct support staff, monitoring service delivery, and promoting positive outcomes for residents. This role requires strong leadership, excellent communication skills, and a commitment to person-centered care.
Resident Services & Case Coordination
- Complete comprehensive resident assessments and ongoing evaluations.
- Develop, implement, and monitor Individual Service Plans (ISPs) and related support documentation.
- Coordinate services with case managers, guardians, healthcare providers, mental health professionals, and community resources.
- Monitor resident progress toward goals and update plans as needed.
- Participate in admission, discharge, and transition planning.
- Respond appropriately to resident concerns, incidents, and crisis situations.
Staff Supervision & Leadership
- Supervise, mentor, and support Direct Support Professionals (DSPs) and Lead Staff.
- Conduct staff training, coaching, and performance evaluations.
- Ensure staff compliance with agency policies, procedures, and 245D requirements.
- Assist with scheduling and staffing coverage as needed.
- Promote a positive team environment focused on resident-centered care.
Compliance & Documentation
- Ensure compliance with Minnesota 245D licensing standards and organizational policies.
- Review documentation for accuracy, completeness, and timeliness.
- Conduct quality assurance reviews and monitor service delivery.
- Assist with audits, licensing visits, and corrective action plans.
- Maintain confidential resident records in accordance with HIPAA and state regulations.
Program Management
- Monitor resident health, safety, and well-being.
- Support behavioral interventions and positive support strategies.
- Collaborate with interdisciplinary teams to ensure continuity of care.
- Participate in on-call responsibilities as assigned.
- Assist in developing and implementing program improvements.
Qualifications Required
- Bachelor's degree in Human Services, Social Work, Psychology, Sociology, Behavioral Science, or related field; or qualifications that meet Minnesota 245D Designated Coordinator requirements.
- Minimum 2 years of supervisory experience in a human services, residential, or healthcare setting.
- Minimum 2 years of experience working with individuals with mental health diagnoses and/or substance use disorders.
- Knowledge of Minnesota 245D regulations and person-centered service delivery.
- Experience completing assessments, support plans, and related documentation.
- Strong leadership, organizational, and communication skills.
- Valid driver's license and acceptable driving record.
- Ability to pass required background studies.
Preferred
- Experience in Community Residential Services (CRS) or Adult Foster Care settings.
- Experience managing behavioral challenges and crisis intervention.
- Training in Positive Supports, Mental Health First Aid, CPR/First Aid, or related certifications.
- Experience working with individuals who have co-occurring mental health and substance use disorders.
Knowledge, Skills, and Abilities
- Strong understanding of mental health conditions, substance use disorders, and recovery-oriented practices.
- Ability to supervise and develop staff effectively.
- Excellent documentation and assessment skills.
- Ability to prioritize multiple responsibilities and meet deadlines.
- Proficiency with electronic health records and Microsoft Office applications.
- Ability to maintain professionalism and confidentiality.

Pay: $22.00 - $25.00 per hour
