Job Summary
Lamar Estates Long Term Care is seeking a skilled Physical Therapist (PT) to join our rehabilitation team. The PT will evaluate residents, develop treatment plans, and provide therapy to improve mobility, strength, balance, and overall functional independence.
Responsibilities
Evaluate residents and develop individualized therapy plans
Provide therapy to improve mobility, strength, balance, and endurance
Assist residents in regaining functional mobility and independence
Collaborate with the interdisciplinary team on care planning
Educate residents and families on safety and mobility techniques
Maintain accurate and timely documentation
Qualifications
Licensed Physical Therapist in the state of Colorado
Graduate of an accredited Physical Therapy program
Experience in skilled nursing or rehabilitation preferred
Strong communication and teamwork skills
