Vantage Hospice is growing. We are seeking an two experienced and compassionate RN Case Managers
The position reports to: Clinical Manager
Qualifications:
- Current licensure/registration as a professional registered nurse in TX
- Previous experience in hospice nursing is preferred.
- Experience in caring for terminally ill patients/families with knowledge of symptom control and emotional support is critical.
- Experience in Oncology Nursing, Home Health, or long-term care nursing is a plus.
- Excellent physical and psychosocial assessment skills, reasoning, deduction, and critical thinking skills are a must. Demonstrated independence in clinical assessment is required.
- Hospice Case Managers must possess the ability to prioritize urgent needs presented by patients, families, and facilities. Must be detail-oriented and able to handle multiple and concurrent tasks.
- Candidates must possess proven time management and organizational skills and have flexibility with scheduled hours.
- Professional attitude and demeanor at all times are essential. Must have reliable transportation, a valid driver's license, and the minimum state-required liability automobile insurance or access to reliable transportation.

Responsibilities
- Conduct comprehensive patient assessments to develop individualized care plans for hospice patients.
- Coordinate and manage patient care services, ensuring compliance with HIPAA and medical regulations.
- Collaborate with interdisciplinary teams to facilitate discharge planning and post-acute care.
- Provide education and support to patients and families regarding hospice services and end-of-life care.
- Maintain accurate documentation of patient progress and care interventions in accordance with organizational policies.

Pay: $85,000.00 - $90,000.00 per year
