POSITION RESPONSIBILITIES
- Visit hospitals, nursing homes, assisted living, and clinics.
- Maintain strong relationships with doctors, nurses, social workers, and discharge planners.
- Represent the hospice professionally in the community.
- Provide education about hospice benefits and eligibility.
- Answer common questions about insurance, medications, equipment, and eligibility.
- Coordinate with admissions, nursing, and the office team to start services.
- Explain the referral process to families and providers.
QUALIFICATIONS
1. Hospice liaison experience preferred.
2. Must be organized and detail-oriented.
3. Must be able to communicate effectively orally and in writing.
Job Type: Full-time
Pay: $50,000.00 - $100,000.00 per year
