Full-Time | Office-Based | Bilingual English/Spanish Required | Healthcare & Patient Services
We are looking for a compassionate, organized, and proactive Bilingual Social Worker – Home Health Service Coordinator to join our growing pediatric home health team.
This position is focused on helping patients and families connect with the home health services they need. You will work closely with families, insurance companies, PCP/physician offices, nurses, HHAs, and our internal teams to coordinate care and help remove barriers that may prevent a patient from receiving services.
The ideal candidate is someone who enjoys helping people, solving problems, making phone calls, following up, and connecting the dots. You should be comfortable communicating with different people, managing multiple cases, and taking ownership of getting things done.
Fluent English and Spanish are required because you will regularly communicate with Spanish-speaking patients, families, healthcare providers, and other members of the community.
What You'll Do
- Communicate with patients and families to understand their needs and help connect them with appropriate home health services.
- Help coordinate services for pediatric patients who need Home Health Aide (HHA) or skilled nursing services.
- Work with the Intake and Clinical teams to understand each patient's needs and help move referrals forward.
- Match patients with available and appropriate HHAs, LPNs, and RNs based on the patient's needs, location, schedule, and authorized services.
- Communicate with families regarding services, schedules, staffing, documentation, and other questions or concerns.
- Contact insurance companies and MCOs to follow up on authorizations, service requirements, and issues affecting patient services.
- Communicate with PCP and physician offices to obtain orders, Plans of Care, medical records, and other documentation needed for services.
- Coordinate communication between families, physicians, nurses, caregivers, insurance companies, and internal departments.
- Identify barriers that may prevent a patient from receiving services and work with the appropriate team members to find solutions.
- Follow up consistently on referrals, authorizations, staffing, and documentation so cases continue moving forward.
- Help identify staffing solutions when a case is difficult to staff.
- Coordinate with the Scheduling team to ensure services are scheduled appropriately.
- Maintain accurate case notes, follow-up records, trackers, and other documentation.
- Escalate clinical, authorization, staffing, or family concerns to the appropriate department when necessary.
- Build positive relationships with families and provide compassionate and professional customer service.
- Assist with community outreach and other patient-support activities as needed.
- Perform other duties related to patient service coordination and office operations as assigned.
What We're Looking For
- Associate's degree in Social Work, Human Services, Psychology, Healthcare Administration, or a related field preferred.
- Previous experience in social work, healthcare, patient services, care coordination, case management, customer service, or a related field is preferred.
- Fluent in both English and Spanish — required.
- Strong verbal and written communication skills in English and Spanish.
- Compassionate and comfortable working with children, families, and individuals from diverse backgrounds.
- Comfortable communicating by phone with families, insurance companies, physician offices, nurses, and caregivers.
- Strong problem-solving skills and the ability to connect information from multiple people and departments.
- Highly organized with strong follow-up skills.
- Able to manage multiple patients/cases and prioritize tasks effectively.
- Comfortable making frequent professional phone calls.
- Able to handle sensitive or challenging conversations professionally and compassionately.
- Detail-oriented and able to maintain accurate documentation.
- Comfortable using computers, email, Microsoft Office, and learning healthcare software systems.
- Dependable, proactive, and willing to take ownership of assigned cases.
- Able to work independently while collaborating with a team.
- Travelling is required for this role - Harrisburg and surrounding area.

In this role, you help make sure those pieces come together.
You may speak with a family in the morning, follow up with an insurance company, contact a PCP office for missing documentation, work with our clinical team to understand the patient's needs, and then help identify an HHA or nurse who can provide the service.
Your work helps turn a referral into actual care for a child and support for their family.
