Key Responsibilities
- Manage daily care coordination workflows, client service activities, and administrative support processes
- Coordinate with clients, families, service partners, vendors, and internal teams
- Monitor service schedules, documentation, follow-ups, and operational reports
- Support intake processes, case updates, service referrals, and issue resolution
- Prepare reports, client records, meeting notes, and workflow updates
- Assist with process improvements to strengthen service quality and efficiency
- Ensure coordination activities follow company procedures, timelines, and service standards
What We Are Looking For
Experience
- Experience in care coordination, client services, community services, administrative operations, case support, or service delivery
- Previous team coordination or supervisory experience is preferred
- Experience working with clients, families, service providers, or support teams is an advantage

Pay: $76,000.00 - $102,000.00 per year
