Job Summary:
Promotes the physical and mental health of patients by applying the nursing process. This is done as a member of the interdisciplinary treatment team and working with the Treatment Teams and the Prescribers, with the goal of assisting patients to assume responsibility for their own health.
Essential Duties/Responsibilities:
- Provides on-going assessment of scheduled patients concerning medication and/or health problems. Reports to client, Primary Clinician and Prescriber regarding observations and, where appropriate, links the client to necessary medical services by coordinating patient care with clinics, PCPs and hospitals.
- Provides instructions to patients related to ongoing medical conditions such as allergies, medication, diet, and emergency plans related to ongoing medical concerns.
- Provides consultation to patients regarding healthy lifestyle including diet and exercise.
- Is available to staff for ongoing consultation regarding medical problems and health care of their assigned patients.
- Is available for urgent care appointments as determined by Prescribers.
- Documents services in accordance with Agency policy and requirements.
- All clinical, medical and administrative documentation is completed in a timely and thorough manner according to established standards.
- Consistently demonstrates the ability to take the initiative and to make decisions/choices without direct supervision.
- Provides outreach services to clients where appropriate and deemed necessary by visiting patients in the community.
- Uses appropriate opportunities to educate clients and staff in the areas of health promotion and illness prevention.
- Performs other related job duties as assigned by their Supervisor.
- Attendance is an essential function of this position.

1.Working knowledge of diagnosis, treatment planning, medications, and psychosocial rehabilitation concept.

8. Ability to express or exchange ideas by means of spoken word and to receive detailed information through oral communication. Primary functions include activities in which verbal communication with clients and with others is key to job effectiveness.
9. Is able to tolerate periods of heavy workload or excessive stress.
10. Visual acuity must meet a minimum standard to work with preparing, reading and analyzing data reports, work with computer terminals, etc.
11. Is able to use medical equipment and supplies accurately and administer medication appropriately.
Education and Experience:
Education: Graduation from an accredited LPN or RN program, BSN preferred.
Experience: One to three years’ experience in Health Care with emphasis in Mental Health.
License/Certification: An unencumbered LPN or RN license in the state of NH is required. Special certifications in psychiatric nursing preferred. A valid NH driver’s license is required with current liability insurance which meets Lakes Region Mental Health Center requirements.
Physical Requirements:
- Is able to sustain light physical effort such as frequent standing, walking, bending, stooping, or reaching and/or occasional lifting of items 20 lbs. or more.
- Is able to tolerate periods of heavy workload or excessive stress.
- Must be able to walk, climb and use stairs on a regular basis and in unknown circumstances without accommodations.
