Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor patients’ health by taking vital signs and observing changes in condition
- Administer medications accurately following care plans and HIPAA regulations to ensure patient safety
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
- Support clients with memory care needs including Alzheimer’s and dementia care through specialized techniques
- Use equipment such as Hoyer lifts or other assistive devices to safely transfer residents with mobility challenges
- Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with healthcare standards
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment in accordance with infection control protocols
- Collaborate with healthcare professionals to develop or update personalized care plans for each individual
- Provide emotional support and companionship to foster social engagement and reduce feelings of isolation
Skills
- Proven experience in home & community care or assisted living environments; group home experience is a plus
- Knowledge of patient observation techniques and ability to recognize changes in health status
- Certification in First Aid and CPR required; additional certifications such as HIPAA training preferred
- Strong understanding of medication administration, vital signs monitoring, and patient safety protocols
- Experience working with individuals with developmental disabilities, autism, or aging-related conditions like Alzheimer’s or hospice care
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Excellent communication skills for working effectively with clients, families, and healthcare teams
- Compassionate attitude combined with a proactive approach to caregiving responsibilities
