FT Licensed Social Worker (LSW)
A respected long-term care organization is conducting a confidential search for a compassionate and experienced Licensed Social Worker (LSW). This opportunity is ideal for a dedicated professional who is passionate about advocating for residents, supporting families, and collaborating with an interdisciplinary team to provide exceptional care.
Position Overview
The Licensed Social Worker is responsible for addressing the psychosocial needs of residents, coordinating care services, facilitating discharge planning, and ensuring residents receive the support and resources needed to enhance their quality of life.
Key Responsibilities
- Conduct psychosocial assessments for residents upon admission and as needed.
- Develop and implement individualized care plans with the interdisciplinary team.
- Provide emotional support, counseling, and crisis intervention to residents and families.
- Coordinate discharge planning and transition of care.
- Advocate for resident rights and promote resident-centered care.
- Assist with family meetings and care conferences.
- Coordinate community resources and support services.
- Maintain accurate documentation in accordance with state and federal regulations.
- Ensure compliance with CMS regulations, facility policies, and applicable state requirements.
- Participate in quality improvement initiatives and survey readiness activities.
Qualifications
- Current Licensed Social Worker (LSW) license in the applicable state.
- Bachelor's degree in Social Work (BSW) from an accredited program required.
- Minimum of two (2) years of experience in a skilled nursing, long-term care, rehabilitation, or healthcare setting preferred.
- Knowledge of CMS regulations, resident rights, and discharge planning.
- Strong communication, organizational, and interpersonal skills.
- Ability to build positive relationships with residents, families, physicians, and interdisciplinary teams.
