Job Overview
We are looking for a compassionate and skilled Hospice Registered Nurse to join our multidisciplinary team. The ideal candidate will have a strong background in patient care, with experience in critical care, emergency medicine, or hospital settings. As a Hospice RN, you will deliver personalized end-of-life care, coordinate with healthcare professionals, and support patients and their families through challenging times. Responsibilities
- Provide comprehensive nursing care to hospice patients in their homes or care facilities, ensuring comfort and dignity at the end of life.
- Conduct thorough patient assessments, including vital signs, physiological evaluations, and symptom management.
- Collaborate closely with physicians, social workers, and other healthcare professionals to develop and implement individualized care plans.
- Manage medication administration, symptom control, and palliative interventions aligned with hospice protocols.
- Educate patients and families about disease progression, pain management, and available support services.
- Document patient progress accurately using Electronic Health Records (EHR) systems while maintaining compliance with HIPAA regulations.
- Participate in discharge planning, case management, and coordination of post-acute or home health services as needed.
- Ensure adherence to health regulation policies and quality standards within hospice care delivery.
Experience
Candidates should possess prior experience in one or more of the following areas: critical care (ICU or Level I/II trauma centers), emergency medicine, hospital medicine, medical-surgical units, geriatrics, behavioral health, or home health services. Additional preferred qualifications include:
- Experience with medical terminology and clinical case management.
- Knowledge of hospital experience including inpatient settings and outpatient clinics.
- Familiarity with EMR/EHR systems for documentation and patient record management.
- Background in managing complex cases involving infectious diseases or acute care scenarios.
- Proven ability to communicate effectively with diverse patient populations and collaborate within multidisciplinary teams.
- Understanding of health regulation policies such as HIPAA compliance and utilization management principles.

Pay: $88,000.00 - $94,000.00 per year
