Job Title: Clinical Liaison (LPN/RN)
Job Type: Full-Time
Schedule: Monday–Friday with occasional evening/weekend admissions support

We are seeking an experienced and relationship-driven Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team as a Clinical Liaison. This role is responsible for driving census growth, building strong referral relationships, and ensuring appropriate admissions into our skilled nursing centers.
The ideal candidate is outgoing, clinically knowledgeable, organized, and passionate about connecting patients with quality post-acute care services.
Build and maintain relationships with hospitals, physicians, case managers, and community partners
Generate referrals through daily account management and facility presence
Review and coordinate admissions based on clinical appropriateness
Serve as a clinical resource for referral partners and families
Conduct marketing and outreach activities to support census growth
Participate in care transition planning and communication with interdisciplinary teams
Provide exceptional customer service to referral sources and patients
Track referral activity and maintain CRM documentation
Participate in evening/weekend admission call rotation as needed
Ensure compliance with Medicare guidelines, state regulations, and company policies
Current LPN or RN license in good standing
Previous experience in skilled nursing, hospital case management, admissions, or healthcare marketing preferred
Strong clinical assessment and communication skills
Ability to build professional relationships and work independently
Excellent organizational and time management abilities
Valid driver’s license and reliable transportation required
Proficient with Microsoft Office and computer systems
Skilled Nursing Facility (SNF) experience
Admissions or clinical liaison experience
