Job Summary
We are seeking an experienced Clinical Director to lead and oversee all clinical operations within our home health agency. The Clinical Director is responsible for ensuring high-quality patient care, regulatory compliance, clinical staff supervision, and overall clinical performance.
Key Responsibilities
- Provide clinical leadership and oversight for all home health services
- Supervise and support nursing and therapy staff
- Ensure compliance with CMS, state, and federal regulations
- Oversee patient care planning, assessments, and clinical documentation
- Monitor quality assurance (QA) and performance improvement (QAPI) programs
- Collaborate with the Administrator on operational and clinical goals
- Conduct staff training, education, and competency evaluations
- Support survey readiness and audit compliance
- Review and improve clinical policies and procedures
- Ensure appropriate coordination of care for all patients
Qualifications
- Registered Nurse (RN) license required (active and in good standing)
- Bachelor’s degree in Nursing or related field (preferred; Master’s a plus)
- Minimum 3–5 years of clinical leadership experience in home health or hospice
- Strong knowledge of Medicare home health regulations and PDGM
- Experience with clinical documentation, OASIS, and care coordination
- Strong leadership, communication, and organizational skills
Preferred Skills
- Experience with Homecare Homebase (HCHB) or similar EMR systems
- Strong background in QA/QAPI and regulatory compliance
- Experience managing interdisciplinary clinical teams

Pay: Up to $135,000.00 per year
Work Location: Hybrid remote in Worcester, MA 01610
