Key Responsibilities:
- Patient Evaluation: Conduct thorough evaluations to understand the physical conditions, capabilities, and limitations of our patients.
- Treatment Planning: Develop tailored treatment plans focusing on sustainable healing and rehabilitation goals.
- Therapeutic Implementation: Utilize a variety of therapeutic exercises, activities, and modalities to facilitate effective recovery and function enhancement.
- Progress Monitoring: Assess and document patient progress, adapting treatment plans to maximize recovery.
- Patient & Family Education: Educate patients and their families on treatment protocols, home exercises, and lifestyle modifications to support effective recovery.
- Collaborative Care: Work closely with other healthcare professionals to provide comprehensive and cohesive care.
- Discharge Planning: Manage patient discharge processes to ensure a seamless transition from therapy to home or other care settings.
- Documentation: Keep accurate and up-to-date records of patient care, including evaluations, progress notes, and treatment outcomes.
Required Skills & Qualifications:
- Educational Background: Must have a degree in Physical Therapy from an accredited program.
- Licensure: Current PT license or eligibility for licensure in New Mexico.
- Technical Skills: Strong understanding of anatomy, physiology, and medical terminology related to physical therapy.
- Soft Skills: Mindful communication and interpersonal skills, with a strong emphasis on educating patients and families.
